Like most group benefits programs, benefit programs offered by MetLife and its affiliates contain certain exclusions, exceptions, reductions, limitations, waiting periods, and terms for keeping them in force. Once approve your dentist will contact you with the name of a participating specialist. MetLife dental policies cover an annual maximum of $0per patient for in-network treatment and $0for out-of-network treatment under the copayment plan for the University of Chicago. In-network providers automatically submit electronic claims on your behalf. Once enrolled you can view your Certificate of Coverage for additional details.


MetLife Preferred Dentist Provider (PDP) plan does not provide identification cards. These certificates explain the dental options available for participants in the high or low OPERS dental plans. Veterans who are enrolled in the VA health care system and beneficiaries and their dependents of the Civilian Health and Medical Program of the VA (CHAMPVA) can enroll in the program beginning Monday.
