DentalGuard Insurance is underwritten and issued by The Guardian Life Insurance Company of America, New York, NY. Products are not available in all states. Policy limitations and exclusions apply. Plan documents are the final arbiter of coverage.

This policy provides DENTAL insurance only. Whenever you or a family member needs dental services, you may visit a carefully screened In-Network dentist or any dentist you wish.

Please check your ID card to verify which network you utilize. Disclaimer: A different vendor offers information you are. For Guardian Dental Managed DentalGuard (DHMO) plans, you must choose one in-network DHMO dentist who serves as your primary care dentist. All care must be provided or arranged by your primary care dentist.

If you do not choose a primary care dentist, a dentist in your area will be assigned to you. There is no out-of-network coverage. However, you have more opportunities to save money when you see a participating dentist in the DentalGuard Preferred network.

Participating providers are also recredentialed every three (3) years or more frequently if required by state law.

In the event of a dental emergency, members may contact a customer service representative to be directed to an appropriate provider who can deliver the care needed as soon as possible.
